The Independent Living Center of the Hudson Valley is seeking a motivated and energetic community educator to serve as our Education & Outreach Specialist.  This position provides primary outreach for our Open Doors transition program as well as Person-Centered Planning for participants who transition out of nursing facilities.  The ideal candidate would possess excellent communication skills, experience with outreach and education, and personal experience with disability is always a plus!

Purpose: The Open Doors Education & Outreach Specialist provides essential community outreach for the Open Doors nursing facility transition program and also provides person-centered planning and self-advocacy training for participants.

Reports to: Director of Administration

Responsibilities:

The Open Doors Program provides transition services to Albany, Columbia, Greene, Rensselaer, Schenectady and Schoharie Counties.
Conduct regular and periodic outreach presentations for all nursing facilities in the Albany South region. Ensure that outreaches meet the monthly and annual deliverables of the program.
Conduct general community outreach as needed and appropriate for ongoing awareness and utilization of the Open Doors program.
Conduct Person-Centered Planning and self-advocacy training to participants after their transition to community living.
Develop positive working relationships with nursing facility staff and program stakeholders.
Provide information about relevant services and refer consumers to programs when appropriate.
Collaborate with the Program Lead to determine barriers to outreach and education and identify strategies to meet the objectives of the Education & Outreach statewide program.
Maintain all physical and electronic records in files, and databases.
Expand knowledge of home and community based services through participation in trainings and in-services.
Perform other program-related duties as assigned


Qualifications:

Bachelor degree in Human Services or related field plus 2 years of experience – or – Associate degree with at least 4 years of related experience
Experience coordinating and facilitating group and individual training programs
Excellent customer service and communication skills
Proficiency with Microsoft Windows, Office, and with Zoom, Teams and virtual meeting platforms
Organized and self-motivated
Must be able to provide own transportation within six county region
Personal experience with disability is preferred
Understanding and acceptance of Independent Living Philosophy or willingness to learn about it
