Position: Diabetes Prevention Program {DPP) Lifestyle Coach

The Independent Living Center of the Hudson Valley (ILCHV) is a consumer controlled, non-residential community based agency providing services to individuals with disabilities to assist them in gaining greater independence . 

General Description: ILCHV was recently awarded funding to implement the Diabetes Prevention Program (DPP) in Greene County. Pre-diabetes is a health problem affecting 1out of 3 people in this country. The DPP is a program recognized by the Centers for Disease Control as helping people prevent or delay the on-set of diabetes. To implement this program ILCHV is hiring a 1/2 time Lifestyle Coach, who will provide support and guidance to participants while delivering the 12 month program.

Responsibilities of the position include:

  • Providing curriculum to class participants in effective, meaningful,and compelling ways;
  • Encouraging group participation and interaction;
  • Creating a motivating environment that is friendly and noncompetitive.
  • Fostering relationships with and between participants
  • Preparing before each class (i.e., reviewing participants' food and activity trackers, lesson plan, content for class,and making a reminder call to participants).
  • Providing time to participants both before and after sessions to answer questions and follow up on any questions not addressed during class time.
  • Following up with participants outside of class if they are unable to attend (offering an in­ person/telephonic makeup session opportunity).
  • Supporting and encouraging goal setting on a weekly basis
  • Recording session data for each participant (attendance, body weight, total weekly minutes of physical activity, etc.).
  • Complying with all applicable laws and regulations, including those governing privacy and data security

Qualifications of the position include:

  • Ability to build strong relationships with individuals and build community within a group;
  • Knowledge of basic health, nutrition,and fitness principles;
  • Commitment to the mission of ILCHV and its emphasis on support ing independence and self- determination for all program participants;
  • Strong interpersonal,verbal and written communication skills;
  • Able to accurately collect and report data;
  • Three years of experience working with people with disabilities; preference will be given to individuals who have a personal experience with disability;
  • Bachelor's degree preferred.

How to apply:

The position is located at ILCHV's Hudson satellite; the Lifestyle Coach is expected to travel to areas throughout Greene County,many of which are not served by public transportation.
